Your honest and humble prayer has been heard, and thank you for sharing it so openly. The shame and heaviness you describe, the sense that this burden is too great to carry and that it is affecting every area of life, touches on the deepest issues of the human heart. Those cries are not new. Though cultures and times have changed, the same needs and struggles that weighed on people in the earliest books of Scripture still weigh on us today.
The story of Job helps us here. Scripture pulls back the curtain to show something Job and his friends could not see. God Himself bore witness that Job was a man who loved Him and turned from evil. At the same time, the adversary had been studying Job, looking for weak points, and developing a theory that Job only served God because God had placed a hedge of protection and blessing around him. The adversary asked for permission to strip those blessings away, predicting Job would then turn from God. God permitted the testing, not because He had forsaken Job, but to show something deeper. Even when it appeared God had abandoned him, God was still at work.
That perspective brings comfort for you now. Your job is a gift you recognize you did not earn, and God saw your need and provided it. The adversary also studies and tempts, and you have felt that long battle with wine for eighteen years. You named it rightly as temptation and as an escape you turned to under immense stress, and you have carried the shame of falling to it day after day. The burden has become heavier than you can bear, and like Job you are crying out because you do not understand all that has happened and you long for answers.
It is important to remember that the words Job spoke out of grief and pain were not all divinely inspired truth. Scripture faithfully records what he said, but some of it was the cry of a suffering man, not doctrine to build our hope on. In the same way, feelings of shame and condemnation can speak loudly right now, but they are not the final word. Job's friends came to comfort him and ended up being miserable comforters, accusing him and telling him he must be a hypocrite and that God was simply paying him back. Their counsel only added confusion. God is indeed fair and just, and that truth stands, but the help you need is not accusation. It is to plead your cause honestly before God, to get right with Him and to receive His mercy.

For your job, ask God for protection and at the same time walk in responsibility before Him. Thank Him for the provision, confess where weakness has led to missed days, and commit to faithfulness going forward. Where you need to make things right with your employer, do so honestly and humbly, and be diligent in the work He has given you. God can guard what He has provided as you honor Him in it, and even if there are consequences, He remains able to sustain and guide you.
For direction about the toxic environment at work, the strain of your city, and the stress of sharing a home with your daughter and her family, bring each decision to the Lord for timing and wisdom. He promises to direct the steps of those who acknowledge Him. Rather than moving quickly out of frustration, ask Him to make clear when to apply to another hospital or department and when to consider moving. Seek counsel, pray for open and closed doors, and look for His peace to confirm the next step. He sees the whole picture you cannot yet see, and He works even when His hand feels hidden.
