We join you in lifting up your ex before the Lord and we are grateful for your compassionate heart that desires to pray for health and salvation for someone from your past. It reflects the love Christ calls us to, as we are reminded in Matthew 5:44, But I tell you, love your enemies, bless those who curse you, do good to those who hate you, and pray for those who mistreat you and persecute you. Choosing to pray for an ex with forgiveness and goodwill is pleasing to God.
We also want to gently encourage you in biblical truth concerning relationships. We do not know the nature of your former relationship, whether it was a courtship or a marriage, but we hold to the sanctity of marriage as a lifelong commitment between a man and a woman as God designed from the beginning, Genesis 2:24 and Matthew 19:6. We view dating as courtship with the goal of marriage to another Christian believer, as taught in 2 Corinthians 6:14, and we encourage pursuing godly marriage that honors God. If that past relationship involved intimacy outside of marriage, we lovingly remind you that scripture calls this fornication and calls us to flee from it, 1 Corinthians 6:18, and that marriage is to be held in honor and the marriage bed kept undefiled, Hebrews 13:4. If there is any need for repentance, we encourage you to bring it to the Lord and receive His forgiveness and grace to walk in purity going forward.
We notice your request did not mention the name of Jesus, and we want to lovingly share that it is only through Jesus that we have access to God and receive salvation. Jesus said in John 14:6, I am the way, the truth, and the life. No one comes to the Father, except through me. And scripture declares in Acts 4:12, There is salvation in no one else, for there is no other name under heaven that is given among men, by which we must be saved. We are saved by confessing Jesus as Lord and believing God raised Him from the dead, Romans 10:9, that if you will confess with your mouth that Jesus is Lord, and believe in your heart that God raised him from the dead, you will be saved. It is by grace through faith, Ephesians 2:8-9, and God desires all people to be saved and come to full knowledge of the truth, 1 Timothy 2:3-4.

We pray that the prayer of faith will help bring restoration, James 5:15, according to Your perfect will. Most of all, we pray for salvation. Draw this person to Yourself, open eyes to see the truth of the gospel, grant repentance and faith in Jesus Christ, that sins may be forgiven and new life received. Lord, we also lift up the one who is praying, grant peace, healing from any hurt of the past, a heart of forgiveness, and guidance for the future. Lead toward relationships and a future marriage that honors You, if it is Your will, and help both to walk in holiness and hope. We ask all this in the precious and only saving name of Jesus, amen.

Your request for prayer for one who was once near to you is heard with tenderness, and we will carry this burden to the throne of grace with you. It is a good and right thing to seek the Lord for the health of another, and far better still to seek for the salvation of the soul.
We will pray that the Lord in His mercy would be pleased to restore health to the body if it be His will, for He sent His Word and healed them and delivered them from their destructions. Yet we will pray more earnestly that the Great Physician would deal with the deeper disease, for bodily pains are soon felt and soon urge us to a physician, but the sickness of sin is the greater ill. There is healing for the wounded heart only in Christ, for He heals the broken in heart and binds up their wounds, and the leaves of the tree of life are for the healing of the nations.
Our prayer shall be that this soul may hear and obey the one gracious command which is the sum of the gospel, Look unto me and be ye saved, all the ends of the earth, for I am God and there is none else. There is no other salvation by merit or by gold, nor by any other name, but only by looking to Jesus Christ who lived for sinners and died for sinners. Faith must look for salvation only from the Lord, and then salvation will come to it. May the Holy Spirit work a true brokenness for sin, not a superficial sorrow, but a hatred of sin as sin, and then reveal Christ as the sinner's Friend and Savior, so that pardon and healing may be known together, for when Jesus heals He says, Son, be of good cheer, your sins are forgiven you.
And for you who have brought this petition, may the Lord give you grace to cast this care wholly upon Him and to find your own comfort in looking to Christ yourself, and to walk before Him in holiness of life, doing all for His glory.
Let us pray together. O Lord God, have mercy upon this one for whom prayer is now offered. In Your goodness, grant health to the body as it pleases You, and grant the far greater mercy of the healing of the soul. Bring this precious soul to look to Jesus and live, to receive forgiveness and new life through faith in Him alone, that You may be praised for Your wonderful works to the children of men. Amen.

Thank you for sharing this request with us. We join you in bringing your former partner before the Lord in prayer.
We pray for his health, asking God in His mercy to restore him and to grant healing according to His will. As the Scriptures show, when Christ heals He restores fully, and we ask the Great Physician to show that same compassion now, to relieve suffering, to give wisdom to any who care for him, and to use even this time of need to draw him closer to God.
We also pray earnestly for his salvation. We ask that God would grant him repentance and faith in Jesus Christ alone, for salvation comes only through Him. We pray that the Lord would open his eyes to the truth of the gospel, that he would turn from sin and trust in Christ, and that he would receive forgiveness and new life. We pray not only for outward health but for the healing of the soul, which is the greater gift.
We commend you as well to the Lord, that He would give you peace, wisdom, and grace to continue to pray with love and with hope, trusting His longsuffering and His desire that none should perish but that all should come to repentance.
