We understand that you're going through a time where you need things to change. Firstly, we want to remind you that God is faithful and that His desire is for you to prosper in all things and to be in good health, just as your soul prospers (3 John 1:2, WEB). We are called to "not be conformed to this world, but be transformed by the renewing of your mind, so that you may prove what is the good, well-pleasing, and perfect will of God" (Romans 12:2).
However, we want to address that your request does not mention Jesus Christ, our Savior and Mediator. It is only through Him that we have access to God, and it is in His name that we should pray. As it is written, For there is one God, and one mediator between God and men, the man Christ Jesus (1 Timothy 2:5, WEB). And in no other is there salvation, for neither is there any other name under heaven, that is given among men, through which we must be saved! (Acts 4:12,WEB).

Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up our brother/sister who is seeking change in their life. We declare that no situation is beyond Your power to transform, for with You, all things are possible (Matthew 19:26, WEB).
We acknowledge and submit ourselves to the authority of Jesus Christ and declare that without Him we can do nothing. We ask for Your mercy and grace for those who have not believed and committed their lives to Jesus Christ. This prayer can only be heard because Jesus Christ took our place on the cross and paid the penalty for our sins so we could have access to you God. We ask for those who have never trusted in Jesus Christ to do so now.
We will praise You, O LORD, with our whole heart; We will show forth all Your marvelous works (Psalms 9:1, WEB). We will be glad and rejoice in You: We will sing praise to Your name, O Most High (Psalms 9:2, WEB).
Lord, guide us all to be doers of your word, and not hearers only, deceiving your own selves (James 1:22,WEB). We desire to obey you and apply your word to our lives. We ask for the strength to obey Your Word and for You to show us the areas where we need to change. Reveal to us any wickedness in our own ways and lead us in the way everlasting (Psalms 139:24, WEB).
We pray for a renewed mind, a transformed heart, and a strengthened spirit to face the challenges ahead.
