We understand that you're going through a difficult time in your relationship and we're here to pray with you and offer biblical guidance. First, let's remember that God's desire is for us to be in relationships that honor Him and align with His principles.
In your prayer request, you didn't specifically mention Jesus. It's important to remember that Jesus Himself said I am the way the truth and the life. No one comes to the Father except through me. All prayers should therefore be addressed to God the Father in Jesus' name, as Jesus is the one who provides access to God the Father.
It's also crucial to consider God's purposes for relationships is marriage. If we possibly can, we should only allow ourselves to be in courtship of another Christian believer, which has the goal to realize if we will be married or not.

Dear Heavenly Father,
We come before You in the name of Jesus, lifting up our sister who is seeking restoration in her relationship. We pray that You would guide her and the other person in this relationship to seek You first and to treat each other with kindness and respect. We ask God that any immoral behavior is stopped immediately and both persons are brought to pursue a relationship that honors God. We pray Father that if this relationship is not able to honor God that it would be ended in Jesus' name. If they are not already married, we pray that they will be able to determine if marriage is their God given path together and only do so in a God honoring way.
Lord, we also pray for their spiritual growth and that they would both draw near to You, seeking Your will for their lives. We ask that You would bring healing, forgiveness, and understanding to this situation and that You would give wisdom on how to turn their relationship to a God-honoring one.
Father God, we pray for this person who submitted this prayer. We ask Father that you would forgive them of their sins and that they may be saved if they have not already been so.
