Please help me stop self-destruction

Dear Lord, please help me to stop. I have been committing the same mistake every year over ### years now. This always gets me in troubles and affect my health. I am afraid that if I do not stop, these mistakes will eventually take my life.

Please help me Lord, for I am tired, stressed, and weary.
