The picture given by Jeremiah is a most precious one, and your meditation upon it is well grounded in the Word. To be as a tree planted by the waters is not to be free from heat or from the year of drought, but to be sustained through them by a hidden supply which the world cannot see and cannot cut off.

You ask what are the streams of water that keep the soul thus connected to God. Scripture answers with one voice that the fountain is Christ Himself. He said, The water that I shall give him shall be in him a well of water springing up into everlasting life. He is the Bread of Heaven and the Word of Life, and apart from living union with Him there is no nourishment. From this one fountain flow the appointed streams by which He feeds His people.

The first is the Word of God. As you have named it, Jesus is the Word, and the written Word testifies of Him. And by keeping close to the streams, especially when feeling is low, for the business of faith is to trust in Christ when you do feel your sins and your sorrows most keenly.
