Firstly, let's remember that God's desire is for us to be reconciled to Him and to one another. As it is written in 2 Corinthians 5:18-19 WEB, "But all things are of God, who reconciled us to himself through Jesus Christ, and gave to us the ministry of reconciliation; namely, that God was in Christ reconciling the world to himself, not reckoning to them their trespasses, and having committed to us the word of reconciliation."
However, it's crucial to ensure that your relationship aligns with biblical principles. If this relationship is a courtship intended to lead to a godly marriage, we wholeheartedly support praying for its restoration. If not, we encourage you to reevaluate the relationship and seek God's will for your life.
Regarding forgiveness, Ephesians 4:32 WEB says, "Be kind to one another, tenderhearted, forgiving each other, just as God also in Christ forgave you." Remember that forgiveness is not just an action but also a heart attitude. Pray for the grace to forgive as Christ has forgiven you.
Concerning healing, Cast all your anxiety on him, because he cares for you. (1 Peter 5:7 WEB) Also, remember that (Psalms 147:3 WEB) "He heals the broken in heart, and binds up their wounds." God is close to the brokenhearted and is able to heal and restore you both.
However, you did not mention the name of Jesus Christ, Jesus, or Christ or an abbreviation such as IJN (In Jesus Name) in your prayer request. It is only by the name of Jesus that we have access to God. Also, there is no other name by which we can be saved. (Acts 4:12 WEB) "There is salvation in none other, for neither is there any other name under heaven, that is given among men, by which we must be saved!" Confess your sins to Jesus and accept him as your Lord and Savior, then you can come boldly to the throne of grace and mercy in time of need. (Hebrews 4:16 WEB)

Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up this beloved person and their relationship. We ask for Your forgiveness where they may have sinned against You, and we pray for healing and reconciliation in their relationship, according to Your will.
Lord, we know that You desire unity and reconciliation. We pray that You would work in their hearts, drawing them closer to You and to each other. Help them to forgive as You have forgiven them, and to love as You have loved them.
We ask for Your healing touch upon their lives. Mend their broken hearts, bind up their wounds, and restore their souls. Help them to cast all their anxieties on You, knowing that You care for them.
In Jesus' name, we pray for salvation for any who do not know Jesus. We pray for a repentant heart, a steadfast faith, and a willing spirit to follow You, Lord.
Guide them in Your truth and teach them Your path. Help them to walk in Your light and to live according to Your word.
