We understand that you're facing a struggle with lust and masturbation, but we want to remind you that you are not alone in this battle. The first step is to acknowledge and confess your struggle in order to overcome it.
God tells us to "Flee sexual immorality. Every sin that a man does is outside the body, but he who commits sexual immorality sins against his own body." (1 Corinthians 6:18 WEB) Sexual sin is a sin against our own bodies, and it is important to flee from it.
The Bible tells us, "For this is the will of God: your sanctification, that you abstain from sexual immorality, that each one of you know how to possess himself of his own vessel in sanctification and honor, not in the passion of lust, even as the Gentiles who don't know God" (1 Thessalonians 4:3-5 WEB). God's will is for us to be sanctified and to abstain from sexual immorality.
We must also put on the whole armor of God, so that we may be able to resist temptation. "Put on the whole armor of God, that you may be able to stand against the wiles of the devil. For our wrestling is not against flesh and blood, but against the principalities, against the powers, against the world's rulers of the darkness of this age, and against the spiritual forces of wickedness in the heavenly places." (Ephesians 6:11-12 WEB)

Dear Jesus,
We come before you, confessing and repenting of the sin of lust and masturbation. We ask for your forgiveness and your strength to overcome this temptation. Help us to flee from sexual immorality and to walk in purity and freedom.
Help us to put on the whole armor of God, so that we may be able to stand against the wiles of the devil. Give us the strength to resist temptation and to walk in your will.
Help us to fix our eyes on you, Jesus, and to run the race that is set before us. Give us the power of the Holy Spirit to live a life that is pleasing to you.
In your mighty and precious name Jesus, we pray. Amen.
