We understand your concerns about your myopia and the other issues you've mentioned. First, we want to assure you that God cares about every detail of your life, including your health and your father's well-being. It's wonderful that you acknowledged the name of Jesus, as it is by His name that we receive salvation and have access to come to God.
In the Bible, we are reminded that our bodies are temples of the Holy Spirit, and we should take care of them.
Or do you not know that your body is a temple of the Holy Spirit who is in you, whom you have from God, and you are not your own? For you were bought with a price. Therefore glorify God in your body and in your spirit, which are God's. I Corinthians 6:19-20 WEB
Concerning your myopia, we pray that God will touch your eyes and reverse the condition. Trust that He is able to heal you and that nothing is impossible with Him.
Jesus looked at them and said, With men it is impossible, but not with God, for all things are possible with God. Mark 10:27 WEB
Regarding your cellphone usage, it's important to remember that God calls us to be sober and self-controlled. If your phone usage is becoming an addiction, it may be helpful to set boundaries and seek God's help to overcome it. Also, pray for guidance on this issue.
But the fruit of the Spirit is love, joy, peace, patience, kindness, goodness, faith, gentleness, and self-control. Against such things there is no law. Galatians 5:22-23 WEB
For your father, we pray that God will grant him strength, good health, and provision. We also pray that He will bless your father's work and that He will not grow weary.

Lastly, it's commendable that you recognize your sins.
We all have sinned, and fall short of the glory of God. Romans 3:23 WEB
God is faithful and just to forgive us our sins and to cleanse us from all unrighteousness.

Dear Heavenly Father,
We come before You, lifting up Your child who is in need of Your healing touch. Lord, we ask for healing of the myopia, and we pray that You will guide the doctors and medical treatments. We also pray for deliverance from cellphone usage that may not be pleasing to You. Help Your child to be self-controlled and to use their time wisely.
Father, we also lift up Your child's father to You. We ask for Your blessing, provision, and strength for him. Help him to continue to work diligently, but also provide the rest that he needs.
Lord, we confess our sins to You, and we ask for Your forgiveness. Cleanse us from all unrighteousness. Help us to live lives that are pleasing to You.
