We are truly glad to hear you acknowledge your need to return to the LORD, as it is written in 1 John 1:9, "If we confess our sins, he is faithful and righteous to forgive us the sins, and to cleanse us from all unrighteousness."
First, we must address the fact that your prayer request does not mention the name of Jesus Christ. It is only through the name of Jesus that we have access to God the Father, for it is written in John 14:6, "Jesus said to him, I am the way, the truth, and the life. No one comes to the Father, except through me."

Dear Heavenly Father,
We come before You in the name of Jesus Christ, lifting up our brother who has acknowledged his backsliding and seeks restoration. Lord, You are faithful and just to forgive his sins and cleanse him from all unrighteousness. We pray that he would turn wholeheartedly to You, seeking Your face and Your will for his life (1 Chronicles 16:11).
We ask for Your help as he endures trials, knowing that You allow trials for our refinement and to draw us closer to You (1 Peter 1:7). Grant him Your protection, guidance, and provision, and deliver him from the schemes of the devil and his enemies (2 Thessalonians 3:3).
Expose and confound any witchcraft or plans of the enemy against him, for You are greater than any power of darkness (1 John 4:4).
We also pray for his physical healing, knowing that You are able to heal all our diseases (Psalm 103:3).

As you return to the LORD, remember the words of Jesus in Matthew 11:28-30, "Come to me, all you who labor and are heavily burdened, and I will give you rest. Take my yoke upon you, and learn from me, for I am gentle and lowly in heart; and you will find rest for your souls. For my yoke is easy, and my burden is light."
Continue to seek the LORD with all your heart, and He will be found by you (Jeremiah 29:13). Trust in His love for you, which He demonstrated by sending His Son to die for your sins (Romans 5:8). As you return to Him, walk in obedience to His Word, and you will experience the abundant life He has for you (John 10:10).
