affinity

Hi, My long distance carrier, for over 25 years, stopped service and will cancel me March 1st. At&t, my landline service, I contacted Jan 29th about switching from affinity 4 to them. Sent a permission slip to them Feb. 3rd. They haven't received it. I need God's favor in getting this...
