We are so glad you have reached out for prayer and support. Firstly, we commend you for your honesty and for seeking help. Addiction, especially to lust, can have devastating effects on one's life, but there is hope and freedom in Jesus Christ.
Let's remember Jesus' words in John 8:36 (WEB), "If therefore the Son makes you free, you will be free indeed." and in Matthew 5:28 (WEB) "But I tell you that everyone who looks at a woman to lust after her has already committed adultery with her in his heart." True freedom comes from Him, and He is able to break even the strongest bonds of addiction.
It is only the name of Jesus Christ by which we can be saved. It is good that you ended your request with Please Jesus help me. This demonstrates your faith in Jesus. But, we must believe in Him and confess our sins. Repentance is necessary.

Dear Heavenly Father,
We come before You today, lifting up this dear soul who is struggling with addiction to lust. We thank You for their honesty and their desire to be free. Lord, we know that this battle is not against flesh and blood, but against the rulers, against the authorities, against the world rulers of this darkness, against the spiritual forces of wickedness in the heavenly places. (Ephesians 6:12 WEB).
We pray that You would give them the strength to resist the devil and command him to flee from them as it says in James 4:7 (WEB), "Be subject therefore to God. But resist the devil, and he will flee from you.". Help them to put on the whole armor of God, that they may be able to stand against the wiles of the devil. (Ephesians 6:11 WEB)
Father, we ask that You would fill their heart with Your love, peace, and joy. Help them to fix their eyes on You and to run the race with endurance. (Hebrews 12:1-2 WEB) Guide them to a local church where they can find support and accountability from fellow believers in Christ Jesus our Lord.
In Jesus' name, we pray for complete deliverance and healing. Amen.
