We are truly sorry to hear about the struggles you are facing, and we are here to pray and offer biblical guidance. First, we must address that the request mentions a relationship, but it is not clear if this is a marital relationship or not. If the relationship is a dating relationship, we must understand if it is a godly courtship with the intent of marriage or if there are behaviors against God's will occurring in this relationship. God's design for relationships is clear: "Flee sexual immorality! 'Every sin that a man does is outside the body,' but he who commits sexual immorality sins against his own body." 1 Corinthians 6:18. If the relationship is a marriage, we will pray for restoration.
If the relationship is not a marriage, then we must consider if the other person in the relationship is also a believer in Jesus Christ.
Concerning the healing from alcoholism, we know that God is able to heal and deliver you from this bondage. Cast all your anxiety on him, because he cares for you. (1 Peter 5:7).

We come before you in the name of Jesus, lifting up this dear soul who is in need of your touch. Lord, if the mentioned relationship is a godly marriage, we pray for restoration and healing. Guide both individuals involved and help them to be a reflection of your love and commitment.
If this relationship is not a marriage, we pray for clarity and discernment. Help them both to lead Godly lives, and if this relationship is not centered around you and your will, give them the strength to end it and wait on your perfect timing and will.
If alcoholism has taken control, we pray for deliverance. We know, Father, that in your strength, you can heal and provide the power to overcome any bondage.
In all of this, Lord, let your will be done as you are the only one with the power to make things right. In the Holy name of Jesus Christ, we pray. Amen.
