We are truly sorry to hear about your situation and we are here to lift you up in prayer and provide some biblical encouragement.
Firstly, we acknowledge your faith in crying out to Lord Jesus Christ. It is only by His name that we have access to God the Father, and it is only by the name of Jesus that we can be saved. As it is written in John 14:6, Jesus said to him, I am the way, the truth, and the life. No one comes to the Father, except through me.
In your struggle, remember that God is always with you and He will never leave you. As it says in Hebrews 13:5, Be content with such things as you have. For he has said, I will in no way leave you, neither will I in any way forsake you.
It is understandable that you feel tired and hopeless, but we encourage you to keep trusting in the Lord. As it says in Proverbs 3:5-6, Trust in Yahweh with all your heart, and don't lean on your own understanding. In all your ways acknowledge him, and he will make your paths straight.
Regarding your health and potential homelessness, we pray that God will provide for your needs and guide you to the right resources. It is good that you were advised to apply for disability assistance. We encourage you to take that step and trust that God will open the right doors for you.

Dear Heavenly Father,
We come before You today in Jesus name, lifting up our dear brother/sister who is in need. Lord, You see their struggles and You know their pain. We pray that You will comfort them, provide for their needs, and guide them to the right resources. We pray for their health to improve and for a safe place to live.
Lord, we also pray that You will strengthen their faith and help them to trust in You even more. We pray that You will make a way where there seems to be no way.
